Support, education and assistance for family members and individuals who provide care for adults over the age of 60 or for any adult diagnosed with dementia. Services are available in Carver, Hennepin, Ramsey and Scott Counties.
Services:
- Comprehensive annual assessment to determine needs
- Individualized plan of care
- Information, education and referral
- Caregiving coaching, consultation and ongoing support
- Individual counseling
- Facilitation of family meetings
- Volunteer respite service
Responsibilities/Fees:
Sliding fee scale for contributions. No one will be denied service for an inability to pay.

Monday – Friday, 8 a.m. – 5 p.m.
