Intake/Case Manager - FT
$28,575/yr to $35,150/yr

Summary

  • Conducts all screening and intake of potential residents.
  • Receive payments, complete move-in/move-out process and update databases.
  • Assist management with Unit Inspections.
  • Assist the Billing Representative in maintaining records of rend and security deposits.
  • Conduct all services in a confidential manner, recording pertinent data and assessing for appropriate referrals and appointments.
  • Provide assessment services aimed at identifying residents’ barriers to successful stabilization.
  • Inform new residents of all rules and policies to establish client expectations within the program.
  • Coordinate service planning and consult with team members to evaluate client goals and outcomes.
  • Maintain and audit case records and files according to established agency policies and procedures, county and state regulations.
  • Provide necessary front desk coverage when needed.
  • Enforce lease provisions and/or rules. Assist the Program Manager in evaluating appropriate consequences for infractions.
  • Respond to and submit incident reports for all instances of chemical abuse, theft, threats and violations of program rules.
  • Use conflict resolution and de-escalation skills to defuse crisis situations. Facilitate social skills and empowerment principles with residents in a group or on an individual basis to promote client growth.
  • Serve as a Mandated Reporter.
  • Provide statistical and data reports as requested by the Program Manager to evaluate and monitor program effectiveness relating to staff and residents.
  • Participate in all required training to meet agency training standards.
  • Notify proper staff of any maintenance concerns and/or safety issues that could negatively affect service delivery.
  • Participate in Continuous Quality Improvement and Safety Committee meetings.
  • Ensure clean up of work areas at the end of each shift, restocking supplies when necessary.
  • Assume other duties as directed by the manager.
  • Proper referrals for chronic shelter users, detox, and/or treatment centers utilizing Catholic Charities Level IV Board and Lodging facilities.
  • Ability to drive an agency vehicle with a valid drivers license and acceptable driving record.
  • Ability to drive a personal vehicle with a valid drivers license and acceptable driving record.

Minimum Qualifications

  • Must be able to pass criminal background check in order to work with vulnerable adults per Minnesota State Law and notify Program Manager of any and all activities that could potentially affect criminal background.
  • Computer competency or computer skills development plan.
  • Ability to read and write to communicate with supervisors and staff.
  • B.A. Degree in Social Work or related field (experience may be substituted for degree). With B.A., minimum of three (3) years experience relevant to the job. Without B.A., minimum of five (5) years experience relevant to the job.

Youth Counselor - On Call

Multiple Programs
$11.00/hr to $12.95/hr

Summary

  • Provide nurturing care, support and guidance to youth in the program.
  • Assist in planning activities and programs in accordance with child’s treatment plan.
  • Plan and facilitate groups.

Shelter Advocate
$12.25/hr to $14.95/hr

Summary

  • This position works as part of a team of shelter advocates to develop a service plan to actively support families' transition from homelessness to stable living. Primary functions includes:
  • Welcoming new participants and developing positive and engaged working relationships with the families.
  • Creation of a client directed Family Action Plan that proactively engages residents in program participation.
  • Assess an work to eliminate barriers to stable housing.
  • Involvement in weekly case review to share progress updates and problem solve with the team on solutions to
  • any challenges impacting the families’ stability.
  • In partnership with Housing First advocate, manage resident files with all necessary case notes and forms.
  • Follow program guidelines for safety in community living (physical safety, safety of sobriety, emotional safety and safety of children).
  • Participate in conflict resolution, restorative justice and success planning with residents for issues that put their Family Service Center program stay at risk.
  • Keep statistics for program outcomes and monthly, quarterly and yearly reports.

Minimum Qualifications

  • BA degree and 1 year experience working directly with families or individuals living in poverty OR without degree 5 years of job related experience.
  • Understanding of generational poverty and how it affects people seeking services;
  • Ability to work with persons with diverse backgrounds--including but not limited to gender, ethnicity, religion, language, culture, sexual orientation, socioeconomic background and/or status, family status, educational background, age, and physical and/or intellectual/mental ability.
  • Excellent interpersonal and engagement skills; effective written and oral communication skills; strong organizational skills; computer literacy;

Program Manager I - FT
$34,220/yr to $42,435/yr

Summary

  • Provide supervision to program staff and monitor operations during the various hours that the program operates
  • Recruit, hire, train, empower, supervise and terminate program staff according to Catholic Charities policies and procedures
  • In conjunction with the Catholic Charities Property Management Dept, provide and maintain Program facilities, equipment and supplies
  • Assist in the development, management, and interpretation of the Program budget and financial reports
  • Responsible for program development, management and evaluation n conjunction with the Program Manager
  • Develops and utilizes Empowerment principles and supports the use of Restorative Justice and Harm Reduction within the program

Minimum Qualifications

  • B.A. plus 5 years experience in Human Services or Emergency Services Field. Without college degree, must have 8 to 10 years experience in human service field including at least 5 yeast experience in emergency housing/shelter programs
  • Must also have 3 years experience in program development, coordination, and staff supervision

Chaplain - FT
$31,345/yr to $38,555/yr

Summary

  • Assist in planning programs & activities at SJHC and other Catholic Charities program sites.
  • Call forth gifted people within and outside the agency(s) to share their gifts; model wholesome living for children/youth, clients, staff and volunteers to witness.
  • Prepare for and conduct regular ecumenical services appropriate to the varied background of the residents/clients; plan and carry out additional services for seasonal events and special occasions.
  • Visit residents/clients regularly and participate in community life; meet with residents/clients or staff as needed.
  • Consults with therapists, family and social workers on individuals progress in spiritual care program; inform staff and residents/clients about Spiritual Care activities.
  • Practice cultural sensitivity in serving residents/clients recognizing the diversity of populations served.
  • Provide resources and guidance for residents/students & staff; recruit, train and support volunteers as needed, assist residents/students who wish to participate in religious education in conjunction with faith communities; support a residents wish to stay connected with their home house of worship.
  • Recruit, train and supervise volunteers to properly staff various programs & activities offered. Periodically review volunteer contributions for effectiveness and progress.
  • Utilize periodic Spiritual Assessment Tool to track residents/students progress; keep copies of active volunteer applications on file.
  • Prepare and file reports, purchase orders, check requests, reimbursements and other internal record keeping tools as needed. These records will be reviewed with and accessible to the Spiritual Care Director.
  • Be available to meet with staff over personal concerns on a short-term basis; refer staff for further counseling as needed; Attend 16 hours of paid training per year through workshops, conferences, seminars or classes; seek spiritual direction, prayer and retreat as needed.
  • Organize and conduct special celebrations and projects that enhance, uplift and build a sense of community; network with community leaders, religious leaders and/or religious communities.
  • Meet with Spiritual Care Director bi-weekly for supervision, planning and to discuss departmental concerns. Participate in weekly Spiritual Care Team meetings as well as Unit Team Meetings according to area of service.

Minimum Qualifications

  • Masters of Divinity, one unit of CPE and previous experience holding a Chaplain role for 3 + years.
  • Be able to lift toys and supplies up to 20 lbs. on a frequent basis. Be able to lift small children weighing up to 40 lbs. on an occasional basis (up to 33% of the time). Repetitive movements at the waist and shoulders may be required when playing with and/or supervising children
  • After appropriate training at St. Joseph's, be able to perform therapeutic holding on children up to 17 years of age. The Chaplin incumbent must satisfactorily complete instruction in First Aid and Crisis Prevention & Therapeutic Holding. The employee must receive a passing grade in both classes. If the employee does not pass on the first try, he/she will be permitted to take the class again. If the employee is unsuccessful in achieving a passing grade on the second attempt, then the employee is subject to termination of employment
  • Must have valid driver’s license to be able to drive small vans carrying residents.
  • Frequent contact with children is required and therefore, should not pose an infectious disease risk
  • Be able to show good judgment and be able to function on an independent basis.

Summary

  • Assist in all phases of kitchen and dining room work, including meal planning, inventory and preparation.
  • Responsible for the orientation, supervision, and evaluation of volunteers.

Transition Support Specialist - FT
$28,575/yr to $35,150/yr

Summary

  • Conduct client interviews (formal and informal) to identify needs and eliminate barriers to securing stable housing, employment and securing benefits, and their mental and physical health.
  • Create individual action plans in partnership with clients including: steps to eliminate barriers, moving forward strategies and making connections to support networks.
  • Assist clients in accessing and coordinating services that supports their transition.
  • Cultivate relationships with a broad range of service providers and maintain a high to ensure appropriate referrals and to help clients navigate the service delivery system.
  • Work closely with Employment Specialist and Housing First and Rapid Exit advocate’s to coordinate referrals and to support clients’ success in achieving their employment and housing goals.
  • Assist individuals in applying for financial benefits through government agencies and various other community resources.
  • Coordinate the application process for individuals in need of state I.D. or a birth certificate.
  • Advocate on client’s behalf with outside agencies to secure services necessary for stabilization into permanent housing.
  • Actively participate in conflict resolution, restorative justice and success planning with clients for issues that put their individual action plan at risk. Complete all necessary documentation.
  • Actively participate in weekly case review ready to share progress updates and to problem solve with the team solutions to any challenges impacting clients’ stability.
  • Maintain accurate client records and statistics to track program outcomes, barriers, and successes.
  • Provide excellent customer service at all times in a strength-based manner.
  • Adhere and enforce program guidelines for safety in the center (physical safety, safety of sobriety, emotional safety and safety for staff, clients, volunteers).
  • Follow agency protocol for all medical emergencies and fill out proper paperwork for incidences of injury.
  • Log entries into Branch III journal in a strength-based manner.
  • Be an enthusiastic supporter of the Catholic Charities agency mission and guiding principles.

  • Maintain regular and consistent attendance, as it is essential to the operation of the Branch III resource center.
