Hope Street Transitional Living Program

The Hope Street Transitional Living Program provides a home for eight homeless males ages 16 through 22. Youth counselors and a case manager provide services, including basic needs, goal setting, emotional and therapeutic assistance, independent living skills training and recreational activities. Other services include access to St. Joe’s Community Health Clinic, education and employment assistance, family counseling and after-care case management services.

Services:

  • Basic needs (food, clothing, transportation, medical attention)
  • Advocacy
  • Intensive case management
  • Crisis counseling and support
  • Mental health and chemical dependency counseling
  • Independent Living Skills training
  • One-to-one educational assistance

2008 Outcomes:

  • 14 young people were housed
  • 71 percent obtained and maintained employment
  • 86 percent attended school regularly
  • 63 percent of youth who moved out of Hope Street moved into long-term stable housing
